The Golden Better at Home provides non-medical services for seniors aged 65+ to help them live independently longer.

We are now also offering Community connector services:

Seniors' Community Connector - Social Prescribing Program - Golden  Seniors Services

The Social Prescribing Program supports seniors aged 65 plus to access non-clinical, community-based services to prevent or delay frailty by fostering resilience and social support using a comprehensive, strength-based approach.
Social Prescribing is an approach to offering non-medical support that enables older adults to identify, set and achieve personal health and wellness goals. A Community Connector helps to find appropriate local programs and services and provides regular check-ins to ensure clients are on track. Golden Community Resource Society has a Community Connector under the Social Prescribing program supporting older adults in the Golden area.

Eligibility: Any older adult residing in Golden area 65+ who is either experiencing or is at risk of:

  • Frailty

  • Social Isolation and/or Loneliness

  • Barriers to Accessing Community-Based Resources

  • Frequent Trips to Emergency Department or Primary Care

  • Experiencing Life-Limiting Illness (Congestive Heart Failure, Renal Failure, COPD, Diabetes, etc.), End-of-Life, or Palliative Care

There is also a referral form through pathways.

Welcome!

Referrals can come from family, friends, medical personnel or seniors. The program coordinator will do an assessment with the senior to see what support they need. These supports range from:

  • light housekeeping

  • friendly visiting

  • transportation

  • grocery shopping

  • library deliveries

  • prepared meal services

  • prescription pick-up/delivery

  • group activities

  • minor home repairs

  • light yard work

*dependant on contractor availability and resources.

There is a fee for some services, including housekeeping, with a subsidy provided for low income seniors; however many services are free to all seniors.
